The MSI installer doesn't support defining custom folders at this time, but you could try creating a Custom Action that copies the necessary files. Please verify that the license file is located in the following folder:

C:\ProgramData\SAPIEN\LicenseFiles

If not, please copy the file to this location and restart the application.

It is possible that the caching process is failing. Please zip the contents of this folder:

C:\Users\David\AppData\Roaming\SAPIEN\CachedData 2.1\Local Machine

And upload it here:

https://www.sapien.com/support/upload

This way we can take a closer look as to what is happening.

Which product and build is this?

Sometimes the software is not in sync with server after renewing. It usually synchronizes when you update the software, but you can press the X button on the notification to ignore it. It will not affect the use of the product.

It is possible that the caching process was still running. If not, try refreshing the cache in the ribbon (Home->Platform->Refresh Local Cache):

Yes, you can accomplish this by enabling the Collapse regions on load option (Options->Editor):

The executable doesn't merge and extracted files, because the behavior is discouraged by Microsoft and antivirus software. Microsoft recommends using MSI installers. And yes, you can use the MSI installer to include all the dependent files with the executable.
