After a reboot, I could now delete the xxxx.exe.intermediate file. I couldn't before even with a logout. Having now deleted the file, I can now build the form successfully (took a couple of seconds). So after I built the packaged executable, I added a few lines of code and tried to build again. The ...

Uninstall\ReInstall fixed the file associations.
However there was an install error about a problem with MSChart
